Submission Process      
To send specimens, please download and print the New Client Registration Form and fax it to us at (415) 883-3842. We will then send you a starter kit with the necessary supplies and forms to be used for your first shipment.
All supplies, submission forms and FedEx air bills needed to submit your specimens are provided by North Bay Histology Lab. In-house courier service is available for those clients within our courier network area (please call for availability). Specimen bottles should be submitted in zip-lock plastic bags with the submission form in a separate zip-lock bag inside the main bag to protect it from leaks in transit. For clients who plan to send specimens on a regular basis, we will arrange with FedEx to set up a permanent account.
All supplies for specimen submission are free of charge to all our clients. Supplies may be ordered by calling our customer service department at (415) 883-2510 or by sending a request by email. You may also use the Special Request Form when submitting your specimens.
All accounts are billed during the first week of the month for the preceding month's charges.
